*This giveaway is open to registrants who received a CSM Raffle email from a Physical Rehabilitation Network ("PRN") representative. Giveaway will close on January 10, 2025 at Noon PST. All duplicate submissions cast by an existing entry user will be considered invalid. Winners will be chosen at random and will be notified via email and/or phone within 72 hours of the giveaway closing using the contact information provided in this application. PRN will assume costs limited to conference registration, one round-trip airfare ticket, two-night hotel stay, and a discretionary meal/transportation stipend as stated on registration page. Winners may be asked to submit photograph content for use in social media material prior to and during conference attendance. CSM Raffle winner is required to consent to the use of their likeness, name, area of residence, school name, prize amount and other contest information without limitation or further payment.
